Every new surgical robot seems to be called AI-powered or autonomous.
Those labels don't tell you how much the robot actually does.
A robot that follows every command from a surgeon is very different from one that can make decisions on its own.
The Surgical AI Autonomy Ladder, adapted from the framework proposed by Yang and colleagues in Science Robotics (2017), provides a simple way to classify surgical robots by their level of autonomy.
Level 0. No autonomy
The surgeon makes every decision.
The robot is simply a tool.
Level 1. Robot assistance
The robot improves precision and stability.
It never decides what to do.
Most robotic surgery today belongs here.
Level 2. Task autonomy
The robot performs one predefined task independently.
The surgeon supervises throughout.
This is where meaningful autonomy begins.
Level 3. Conditional autonomy
The robot adapts its actions as conditions change.
The surgeon remains ready to intervene at any moment.
Most systems at this level are still experimental.
Level 4. High autonomy
The robot independently performs major parts of a procedure.
The surgeon primarily supervises.
No commercial system has reached this level.
Level 5. Full autonomy
The robot plans and performs an entire operation without human supervision.
This remains a research goal.
Where we are today
Despite the headlines, nearly every commercial surgical robot still operates at Level 0 or Level 1.
A handful of research systems have demonstrated Level 2 capabilities.
Levels 3 through 5 remain largely confined to the laboratory.
