Ottava vs. da Vinci 5: the first real challenger in 20 years

A fundamental design break

Da Vinci 5 is an evolution. Ottava is a reset.

Intuitive still wheels in a cart and tower, positioned and calibrated before each case. Ottava builds four arms into the table itself, hidden until needed. No cart. No external footprint.

J&J claims a 30 to 50 percent smaller OR footprint. Setup goes from multi-step to nearly one button.

Workflow vs. perception

Ottava's bet is twin motion. Table and arms move together. Reposition the patient, and nothing has to unlock or recalibrate.

Da Vinci 5's bet is Force Feedback. Surgeons feel the tissue at the instrument tip. Intuitive says it cuts applied force by up to 43 percent.

One optimizes for workflow. The other optimizes for feel. Both are serious bets on outcomes.

What each is cleared for

Ottava: ten upper-abdominal procedures. Gastric bypass, sleeve gastrectomy, cholecystectomy. Focused and general-surgery-first.

Da Vinci 5: two decades of indications across urology, gynecology, general surgery, thoracic, and now cardiac. Over 12,000 procedures in six months. 20 million patients treated on the platform overall.

Ottava is starting where da Vinci started. Narrow, then expand.

What actually matters

For the first time in twenty years, hospitals have a real choice. Not a pilot system limping toward relevance. A legitimate alternative built on a different idea of what the OR should be.

Intuitive will keep expanding indications. J&J will keep pressing its workflow edge. Neither will sit still.

Competition has arrived in robotic surgery. Every hospital's robotics roadmap just changed.
